Paperwork that MUST be Completed Before Tryouts
1. Complete athlete Registration in Medina FinalForms: medina-oh.finalforms.com
2. Athlete’s Sports Physical:
a. Parents complete their portion of the physical via Final Forms;
b. Print out a copy from the FinalForms Account;
c. Your child’s pediatrician must then complete, stamp/sign & date their portion of the
Physical.

7th Grade: 7:30-9:30am
8th Grade: 10:00am-12:00pm

@Claggett MS, Athletic Doors at Bus Loop
Monday August 3rd
7th Grade: Time TBD

AI Root Tryout Dates
@Medina HIGH SCHOOL Gym, B3 Doors
Saturday August 1st
8th Grade: 12:30-2:30pm
7th Grade: 2:45-4:45pm

@AI Root MS, Athletic Doors by Cafeteria
Monday August 3rd
8th Grade: 10:00am-12:00pm
7th Grade: 12:00-2:00pm

Notes
1. Athletes must plan to attend ALL tryout dates for their school/grade.
2. If your athlete is invited to play volleyball this 2026-27 season, practices begin the day
after tryouts and will continue every weekday in August unless noted otherwise.
3. Schedules will be discussed during Team Parent Meetings and communicated via
FinalForms and on the Medina Athletics website.
